First off, anyone who lists George W. Bush should probably be ignored because:
1) They have NO idea what it is like to live under an actual dictator.
2) No one has yet to provide proof as to Bush's dictator status. They just don't like him.
3) S/He seriously needs to open a history book, look and compare at brutal dictators such as:
Hitler
Stalin
Pol Pot
Hussein
Qaddafi
Castro
Lenin
Mao
Mussolini
Marcos
Kim Jong-Ill
Kim Ill Sung
Duvalier
Note: I know I probably have more than 10. The above are listed in no particular order. Some are not as murderous as others (such as Castro) but are listed simply because of the control they have and thier longevity.

Also, there are many church 'dictators' (popes) of the Catholic church and the protestant church who were very murderous and responsible for millions of people murdered around the world and in Europe. The Catholic church forced people to convert to Catholicism, including Jews, during the Inquisition. They were very brutal dictates and sentences handed down in the name of the church and the pope.
However, my list is in no order, nor is it a comprehensive list. Nor can I discern who is worse. But they are all horrible men and horrible dictators. Look them up if you need to know their background.

Pretty sure as far as number of people killed its Stalin, then Mao Zedong i think, The worst modern dictator would have to be the president of Sudan.
