Concert tickets to a favorite rock band from their youth. Or maybe their favorite sport. If you're in a relationship (romantic), try a getaway and re-kindle the LOVE! Guys can be romantic, too. Just don't get him anything for old people, like diet pills, adult diapers, baldness cures or vitamins! He'll feel lousy enough already!

fn_hot, I think for a man of this age, they usually just about have everything they could ever need.. So, you might want to bake him a nice brown & blue cake, unless his birthday falls on the 4th of July, then red, white & blue cake, and bake it with his favorite cake mix, and icing.. Then get his favorite ice cream as well.. And maybe a gift card to his favorite stores.. Or, if he has any types of hobbies, find out what they are and get something to go along with his collections, or his hobbies.. You could get him maybe something nice for his home, or make him a nice dinner, and bring it and the cake to him as your gift.. Or order out, for him while your there, and buy him a little trinket or two in a 'gift bag' with a card of course, then when the door bell rings, foot the bill, and afterwards have cake and ice cream.. You could see what kinds of cologne or aftershave he uses, and buy him some more, or a nice tie.... Maybe a new travel shaving bag, those sorts of things, a new wallet.. Just go fishing ask his friends what sorts of things he likes, and is into, and so forth.. And go from there.. Good luck, he'll be thankful you even gave him the time to care, trust me!! Smile!!!
