This is tough! I am glad you are so safety conscious. When I was younger and even sometimes now if we are in a shady area (I'm 23), my dad would have me stand a few feet outside the restrooms - maybe between the womens' and mens'. That way, she is in earshot in case she needs to yell fo help and if someone was to grab her, there would be people entering and exiting the bathroom - very public place, so nobody would take that chance and be caught. Hope this helped! And also, it helps that you men only take about 30 seconds!

The answer will depend upon the age of the child.
When my daughter was five or younger and it was neither safe for her to go to use the toilet at a department store, restuarant, etc. by her self, I would take to the men's room, escorting her to the largest stall and assist her.
Now that she is older, I escort her to the ladie's room and wait outside fr her. If she has been in there for what seems to be a long while, I will call out to her to see if everything is all right.
Should I ever hear her respond that she needs help or should I hear her call out that someone is bothering, attemtping to molest or assaulting her, I would enter the room so as to assist her, apologizing to any and all, with the exception being the person or pesons who were bothering/attemtping to molest or harming her. (I would late call for medical assistance for those persons.)
I am reasonalbly certain that I will escort her to the restroom until such time as she demonstrates that in a given situation she would be well able to defend herself. (She will be taking aikido lessons!)

Take her to the mens bathroom . If you go into the womens room and it is occupied it will be a problem

This is what the "family bathroom" was created for. Prepare ahead of time or use newly constructed malls and you'll be able to use the family restroom, outside of the women's AND men's rooms.

Take her into the men's bathroom. If she's too young to wait by herself noone will be offended if she goes in the men's.

Ask a female clerk, or an older shopper to help your little
girl. It shows a lack of regard for personal privacy for you
to take her into a female restroom, and guys restrooms are
usually...well....eecch!
