Before the first pitch it is till the home teams call. This would most likely be placed in the hands of either the grounds crew manager, or the general manager.
After the first pitch, the decision falls to the umpire crew. The head ump would have the final decision, but it would probably be a group decision.

Up until the game actually begins it is the decision of the management of the home team. Once the first pitch is thrown, the authority rests with the umpire-in-chief or crew chief.

The home team can cancel a game before the game starts due to poor field conditions. However, once it reaches game time it's up to the head ump.

Great question. Since the early 1960's (i think), the league has placed authority under the umps to call the games off if weather is questionable.
