Yes, it does happen. Somebody operating such an access point can easily read all of your (unencrypted) traffic. A sophisticated scammer will also employ fake DNS computers to route you to counterfeit servers that look like real destination servers (like Facebook, or Yahoo!). In this manner, they can even get your encrypted traffic. It's also possible that these counterfeit servers could install viruses on your computer that will persist, even after you leave the "free" wi-fi. Practice safe computing: know who you interface with!

It depends on the service. If there is a charge, it usually comes in the form of a web page where you enter billing information before you can surf the web.

You can connect for free, but I'd be wary of any unfamiliar connections. You may just be inviting hackers.
