I know you're looking for a place to sign-up -- the only one I know of is Gold Star Chili (Cincinnati Area) -- but a lot of fast food restaurants will if you ask. A few years ago a friend of mine went through all the local drive-thurs telling them it was his birthday (yeah I know it's dumb) and most gave him a free small fry or a small drink only a few refused him.

Nearly all casual dining restaurants will give you a free dessert if you eat your meal there -- if you ask nicely some will even do it without the entire staff birthday chant.

Gratzies does

BJ Clancy's does

I've heard a rumor that Denny's does, but not sure on that one.

Might not be in your area though. Call around to local restaurants and ask if they offer free birthday dinners and what the catch is.
