my husband and I married in chicago il, like 8 months ago but i didn't take his last name, where wouldI go to do that?

If you choose to take his name, you can take your marriage license to the DMV and get a license with your "new" name on it. That should lay the groundwork for changing it elsewhere.

In any event, your marriage license should be "proof" to anyone (banks, etc) that you're entitled to change your name.

Hi I am from Illinois too. The local post office should have forms for you to send to social security, you will have to go to DMV to get your DL changed. Make sure you have copies of your marriage License.

I'm going through the same thing. I hear you have to go to Social Security and then DMV. Good luck.

I would try court house and social security office.
