It means a position of mutual oral sex. Picture a man on his back and a woman turned the opposite direction on top of him.

You can do it with the man on his back and the woman on top, or also with the couple on their sides facing each other.
The term "69" comes from the way the numbers look together, like a couple having oral sex on each other.
