Most confusing movie ever- it has to be 12 monkeys.
There were so many flash fowards, flashbacks, it should've been called you have to watch this twelve times to catch every bit of foreshadowing in this monkey movie

The Fountain
While it all makes sense by the end of the film (sort of) you spend most of the time in the movie staring at the screen and muttering things like "WTF?"
A romance, maybe, but not all that watchable.

Mission Impossible - The first time I watched it I got confused on who was dead, who was alive, and who was on which side. It took a second time watching to take in all the double-crossings.

The Butterfly Effect was very confusing once I saw it the first time, after watching it again I get what happened. Another one that's really confusing the first time you watch it is The Prestige.
