There are some great choices here.
Alien and Jaws are definitely among the most suspenseful movies ever made. So much time passed without a glimpse of the shark, or a good look at the creature on the ship, plus there was the whole conspiracy aspect. What made those movies work was that anyone who actually SAW something didn't live long enough to tell.
I love the Usual Suspects and the Sixth sense- those are the two best "shocking twist at the end" movies I've ever seen- but they didn't have an atmosphere of tension throughout the whole film like Jaws & Alien- or more recently, the Ring.
Jaws had the biggest impact upon the public and the movie industry. But for a single movie-going experience start to finish, my vote goes to:
*** John Carpenter's the Thing ***
Trapped in Antarctica with certain death from the elements outside, and a killer inside that KNOWS your every move because it's one of your group? No way to trust anyone?
Just the fact that everyone knew they were talking to the killer, people trying read each other's faces and body language looking for some tiny clue, some hint, anything... Best in my book!

Psycho probably, but there have been lots of great suspenseful movies.

North By Northwest
12 Angry Men
The Sixth Sense
Taxi Driver
Deliverance
Misery
Silence Of the Lambs
Rear Window
Fargo
All of these would rank among the most suspenseful movies ever made.

The original Jaws. I think that movie has had more impact in placing fear into our society and culture on a real subject than any other movie. Just look how many people are afraid to go into the water at beach because they fear the sharks. It has become a thing that has affected the entire world by adding much more fear to something that is not nearly as dangerous as we think. Whether you saw it when you were 5 or 50 you were introduced to a new fear and were greatly affected by it.

The final scene in The Grudge (with the chick crawling down the stairs) keeps me on the edge of my seat, even though I've seen it a dozen times.

My first thought is Hitchcock's Rear Window. Then I thought of U-571. I had a knot in my stomach through the entire thing.
