If the shell follows the lines of the truck's cab, it could improve gas mileage a little providing it doesn't weigh too much.
Pickups are inherently not very aerodynamic. The empty pickup bed creates an air vortex that "sucks" the truck backwards. An aerodynamic camper shell helps to move the air to the rear of the truck and prevents it from being trapped by the closed tailgate.
A camper that extrudes above the top of the cab makes it harder for you to move through the air, so it robs you of power and mileage. This type of camper either offsets its benefits, or is a liability based on it's poor aerodynamics and weight.

yes! it will hurt it pretty badly. I thought it would improve because it might make it more aerodynamic. I had a lighter fibgerglass one but my mpg on the highway went from 22 to 17 when I put the cap on. Sold it! Couldnt take it killing the gas mileage. I think it was because of the extra weight, or maybeit just made it less aerodynamic
PS putting the tailgate down will give you about 2 more mpg on the freeway (unless its like a ford triton v10 then nothing helps lol)

Yes. A camper shell can affect gas milage by causing drag.

Yes it will add weight to your to your vehicle hence your MPG will fall. Also it may impact in way of adding wind resistance. I would assume your vehicle will be less aerodynamic with the shell in place.

Yes, do to weight it slightly effects your gas mileage. So does carrying around dead weight such as things that are not used for emergency repairs like extra boxes with books in them and etc.

You lose a bit of gas mileage as they make the truck heavier. If the shell is the type that is the same height as the cab, they are not too bad. Any taller than the cab you lose even more.

It will affect your gas mileage negatively. You have extra weight you are carrying and the aerodynamics of the shell cause more wind resistance.

Man I have read every thread I can find on this and all I have figured out everybody even experts testing theories have different opinions. It is safer to say it depends on too many factors to honestly calculate from one scenario to another.
