They use a BLANK CD (or DVD) - these are NOT the same as CD-R/DVD-R. They use a special type of burner that burns these disks only. It is much more expensive - but the disks last far longer than the typical 2-3 years that most burns are good for

Depends on the System. It could be from a CD-R, DVD-R, to Blu Ray.

Then they mass produce it by making copies.
