Neil Armstrong first flew into space on Gemini 8 with Dave Scott. Their mission was to rendezvous and dock with a target vehicle called an Agena that had been sent up shortly before their launch. They were seccessful, and performed the world's first docking in space.

However, shortly afterwards a stuck thruster on the Gemini capsule caused the docked spacecraft to go into a slow spin. When they disengaged the spin worsened, eventually getting up to one revolution per second. The astronauts were close to blacking out, but Armstrong was able to shut off the thrusters and use another set reserved for re-entry manoeuvres to bring the spacecraft back under control. Mission rules stipulated that once those thrusters were activated the crew must return to Earth, and so Armstrong and Scott ended their planned four day flight just ten hours after launch. This was the first real crisis in space in which the astronauts' lives had unquestionably been in real danger. Armstrong and Scott both received commendations for their clear thinking and calm actions that saved themselves and NASA's reputation.

Armstrong's second flight was of course Apollo 11, in which he, along with Buzz Aldrin, made the first manned lunar landing.

Just the one time, I think. Not sure, he may have been on one of the Gemini missions also.
