When you sell/let a property you are never asked to give information regarding disabled facilities or amenities, thus this info does not exist.

I totally understand where you are coming from. But there is help out there. You can try Adult Social Care to start with. If they do not have the info you need they can usually put you in touch with the right people to talk to. Also try a local library as they have all sorts of info. There is also DIAL, which is a service for people with diasabilities. Here in Cornwall, we have a few housing trusts that can help as well. I dont know if you have anything like that in London?
Basically it's a matter of finding the right people to talk to, which I know from experience, can be tricky.

I never had this problem even though I am disabled but I do recall that in the early days of the NHS every hospital had an someone who's job it was to look after the interests of patients outside the hospital. I suppose this job has been taken over by social services but it may be worth asking at the hospital who deals with your disabilities to see if they have someone like that or can tell you where you can receive such help

Every County Council, not local council, will have a Welfare Rights Officer(s) within their Community Support Division, which will be found within the Adult and Community Services Department. May I suggest that you contact them. May I wish you all the best. Maybe you will also find that you may well be entitled to grants or benefits for the purpose of adaption to purpose as well.
