I would have to say "The Exorcist". Reagan's voice was the scariest part to me!! Yikes!!

Last House on the Left - Do I even need to explain?
Saw - I am normally not afraid of clowns, but every time I see that clown/hear it's voice, I get a little freaked out
Edit: I liked the Exorcism of Emily Rose but I wouldn't consider it a horror movie.

Alien. Whereas the sequals became more sci-fi action, the original was a horror movie. The enclosed environs of the spaceship that gave you no way to escape was the ultimated haunted house with an implaccable, deadly monster roaming loose.
When I first saw it on a full-sized theater screen, in a theater that had only one screen, with 70mm Dolby, and the scene where Dallas is in the air vent. You have the people with the motion sensors who can detect the creature approaching Dallas, but he cannot see it, so you have the audience watching an audience who know the guy is about to get it, then BAM!
I swear my heart stopped.

The Descent was the scariest amongst the recent films.
It is scary because of the scenario:
You are cave diving, you get lost a kilometre under the surface of the earth, there is no light, other than a camcorder than shoots infrared.
There are creatures in the dark that want to eat you for lunch.
It is very scary and it is not handicapped by a bad soundtrack. The realism had people in the theatre on their seats, cause you don't know when these "gollum" like creatures were going to come out of the woodwork (well, stone work in this case).

I am going to list a few that I think are great Horror movies...the reason for listing these is that I like them for being very suspenseful and/or they mess with your mind.
High Tension
Hard Candy
Brainscan
Silence of the Lambs
28 Days Later
Acacia
Reincarnation
You should check these out...they are a must see for any Horror fan.

I find Asian horror are the best, but there are also some like the Sixth Sense, which scared me because I don't like the whole idea about ghosts.
Shutter and The Eye are two of the best Asian horror films I have watched and made me jump out of my seat!

Maybe not the best but the scariest three I have seen are:
Last House on the Left (just disgusting)
Blood Sucking Freaks (just as disgusting)
orginal Texas Chainsaw Massacre (Terrifying)
Last House is all about this could really happen, no monster just messed up urban people.
Blood is just gross and twisted
Chainsaw gave me nightmares for more than a decade! No joke.

Although it may not have been classified as a horror movie, "Backdraft" was one of the most frightening movies I have ever seen because the "monster" (fire) was real, could be identified with by anybody, and can strike almost anywhere at any time... now THAT is frightening!

The Thing... that creature was so awesome it didn't even need a name, in fact... not given it a name made it better.
And I agree w/ Navymom... the daughter screaming for her mother while trapped was darn scary.
(another edit)
but for a movie you probably haven't seen yet...
I suggest The Eye. Its subtitled, but is very much worth watching. There is a The Eye 2, separate story, but good as well. (Not gore, but suspense)
