If you mean Windows 2000 or later, the answer would be "neither". It's loaded as a part of Windows.

If you mean before that, it's a two part process... A device driver for the CD drive is loaded in CONFIG.SYS, then MSCDEX is loaded in AUTOEXEC.BAT pointing to that device.
