The official name for the painting is "La Gioconda" and she is thought to be the wife of Francesco del Giocondo.

"Nobody knows for certain whom Leonardo’s Mona Lisa represents. Is Lisa Gherardini del Giocondo looking at us, or is it Leonardo’s girlish lover, his mother, or is it a compound image, perhaps even a hermaphrodite, representing several persons at the same time? " from http://www.aiwaz.net/MONA-LISA---Sol-and-Anima/a1

Some have even suggested that this is Leonardo's male version of himself.

I agree with people saying that she was Lisa Giocondo, but a woman has come up with a theory that the Mona Lisa is actually Da Vinci as he portrayed himself as a woman. This lady (i forget her name) said that she compared the self potrait of Da Vinci and of the Mona Lisa by flipping the image of Da Vinci and placing one over the other. She claims that the facial features were extremely similar. I don't know whether this is true or not.

Actually she was a real person...
Leonardo Da Vinci painted the Mona Lisa on a piece of pine wood in the year 1506.
Never in the history of Art has one painting been so admired. This is due largely to the enigmatic smile, which has caused much speculation.

One of the most questions surrounding the Mona Lisa is the following: who is she? No one knows for sure. In fact, there have been nearly as many different answers to this question as there are people who have pondered it

There are several theories as to who Leonardo da Vinci's Mona Lisa was. Some believe that she was a real person who da Vinci was enamored with and painted, while others believe that she was an expression of da Vinci's femine side and how beautiful he thought himself to be.

Lisa Gherardini Giocondo (Mona Lisa) was born in 1479 and would have been about 24 years old when Leonardo painted her. Her father was a Florentine Nobel Antonio Maria Di Noldo Gherardini.

Mona Lisa was actually a man that dressed up as a woman and yes, he was real. Now Im not sure if there was ever an actual "Mona Lisa" but all the paintings are of a man.
