Man has created a number of artificial fresh water lakes to allow for flood control and irrigation of dry areas. One such huge lake is Lake Mead which formed behind the Hoover Dam.

If you want to split hairs technically all man made fresh water lakes are reservoirs and not lakes but in common lexicon they are referred to as lakes.

No. As mentioned before there are other man-made lakes too. However, they are not all resevoirs or dammed rivers. Here in Minnesota, you can dig deep enough for a large enough area and you will have a lake. They do this just about every time they build a new subdivision. They do it for asthetic purposes as well as storm drainage purposes. They have to, or the water would have no other place to go except into people's basements.

Absolutely not. Many fresh water lakes are made by damming rivers or other waterways. Some of these artificial lakes can be very large.

No. There are many man-made fresh water lakes. That's what you get when you build a dam.

No. In all of Texas, for example, there's only one lake (if you count reservoirs as lakes) that's natural--Caddo Lake in northeast Texas.
