Well, we're all really confused, that's why I love Lost so much. Hopefully it'll all be cleared up eventually. But basically the last scene between Jack & Kate is a flash forward, something that happens after they get off the island.

Instead of a flash-back, this was a flash-forward. I think they are setting us up to stop thinking, "Oh no! They did something that will prevent them from leaving!", to, "Oh no! Someone found them!"
They said that Lost did not have an ending in sight, and so the writers were just trying to keep us entertained and to keep watching, but now there are only so many episodes left, and the end is in sight. They are starting to get back to the storyline, which I think is better than random adventures.

The answers above are all basically good. For more in depth info you should try www.thefuselage.com. It is run by the creators of lost and the on-line community have a great love for the show. Tons of analysis and plenty of spoilers too, so watch out.

I think it's a flash forward of a "possible senario", maybe if they had gotten rescued at that point. I heard the producers are going to be throwing those in next season as well. I think it's great, always stirring things up and keeping everything mysterious.

Yeah, that scene was a flash forward in time. Yeah, it is confusing but, that's the point the want you to be confused for now and they also want you to think what will happen too. It is sorta a mystery show.
