A 12th man cannot keep wickets nor be a runner. An injured keeper is usually replaced by another player from the playing 11.
On the other hand, a player has to be dismissed himself in order to have him running for someone else.
For a runner, the rival captain's permission is not required; rather, the umpire has to be asked.

by runner always has to be a batsmen who has already got out on that innings (unless running for the opening batsmen).
12th man can only field. he cant bat or bowl.
If a bowler is injured while he's bowling (if the over is not completed), the injured player can be replaced by the 12th man, but the 12th man is not allowed to complete the over. Someone else can complete the over.
If a batsman is retired hurt, he can come back after falling a wicket. but 12th man cant bat for the injured batsman.
To call a runner or substitute 12th man, need to get the permission from the umpire, and also need to inform the rival captain.
