Codes are sometimes different in different areas, but this is fairly standard.
All decks, balconies or porches, open sides of landings and stairs which are more than 30 above grade or a floor below must be protected by a guard not less than 36 inches in height. Open guard and stair railings require intermediate rails of an ornamental pattern such that a sphere 4 inches in diameter cannot pass through.

In the state of Massachusetts, the building code requires a minimum 6'-8" clear for any habitable space. So, if this balcony is inside and used for living space it needs 6'-8" clear. To the best of my knowledge, there is no distinction for an exterior balcony.
Your best bet is to ask your local building department, as each state and in some cases, each city or county may have a different code. Sometimes local inspectors have varying opinions as to how to interpret the code.
So my answer may be fine in Mass., but way off base in Florida. Better to ask ahead.

As Gary suggests the zoning/codes specific to your area may be slightly variant from others, but as you mention a second story, I must assume it also depends in some measure to the ceiling/joist height of your first/ground level. Obviously a 10 ft. ceiling for your first level should help dictate that the second level is plus that, hence defining where the "balcony" exit/entry should be, IE: Floor level of the second story, and be attached via ledgers, etc., and code proper posts/pillars to ground level
