Yes - the link below has a list of the occurrences - for some reason, it's a lot more frequent in the National League than the American League.

Yes, according to the Baseball Almanac, it has happened 39 times( Nine pitches-nine strikes-side retired). Rick Helling was the last one to do it on 6/20/2006. The first one was John Clarkson of the Boston Beaneaters(NL) 06/04/1889. Others of note: Nolan Ryan( one in each league), Sandy Koufax x 2, Bob Gibson, Lefty Grove x 2, Jim Bunning, Ron Guidry, Pedro Martinez, Randy Johnson.

Oh yeah. I don't have a more recent record book than the 2004 edition (so, through the 2003 season), but it lists 11 occurences in the AL and 25 in the NL to that point. I won't put up the whole list, but there's some familiar names: Rube Waddell (first known occurence, 1902), Lefty Grove twice, Jim Bunning, Al "served up Hank's 715th" Downing, Nolan Ryan (once in each league), Ron Guidry, Pedro Martinez, Warren Spahn, Sandy Koufax twice, Bob Gibson, Bruce Sutter, Rob Dibble, David Cone, Randy Johnson.
It's rare but it does happen.

I've seen it before. It was a recording of an old 80's game. Between the Mets and Marlins(not sure it was the Marlins)
"Hit by pitch (first pitch). Then an infield fly(first pitch). Then second pitched into a double play. Then the 1st batter of the next inning hit a shot into left field on the 2nd pitch but it was about 5 feet from the wall. So far its 6 pitches. The next batter hit a pop up to the third baseman or shortstop( i forgot) Then the next batter got a base hit on the first pitch. And then the next batter hit it to the SS with a fielder choice to second base which makes that 9 pitches.

our a.j burnett pitches 13 strikes outs per game. Roy halladay, Dustin mcgowan and shawn marcum...all of them are good at striking out batter one after the other.

Yea, there are complete games where one team only get strike outs. it's called a perfect game. and 9 strikes in a row is called a 1,2,3 inning.

Rick Helling of the Brewers did it last year against the Tigers. I think he struck all three out.

Yes. The only pitcher I can think of is David Cone when he was with the Mets. I'm sure that other pitcher have done it as well.
