I need a excel form that I can customize for my company needs, but basically requires sales reps to report specifics of their weekly sales activities, close rates, and follow-up / opportunities.

You definitely want excel? Have you considered using ACT for the followup? I may have an old one onhand and can check but there is always from for error when you are using a shared excel spreadsheet. if you know anyone from a staffing firm most of us have some onehand. I amy have some "hunting /farming" sheets I could share with you.
