I mean what if they regain consciousness while under, but are to overcome to rise out off the water?

Or do they always stay unconscious the whole time?

Shallow Water Blackout is categorized as an unexpected loss of consciousness that is not regained. This type of drowning incident is usually preceded by hyperventilation as a means of extending breath-hold dives. The body basically takes in so much oxygen during hyperventialtion that the CO2 buildup isnt noticable in the bloodstream until it is too late. Those who experience shallow water blackout never get the urge to take a breath before they lose consciousness and are often found floating on the bottom of the pool. Shallow water blackout occurs quickly and painlessly and often goes unnoticed by others in the vacinity since it is so quick and shows no outward signs.
