To stop doing something immediately, usually a habit which has been very hard to break, eg smoking. I think it has to do with the idea that eating turkey which is cold can be a shock to your system (it's very cold!), just as stopping an old habit immediately can be as well.

It means stopping something you are currently addicted to with no means of lessening it, like cigarettes......
If you smoke, stopping cold turkey means no more, ever.
Most people have that urge or craving for a cigarette, so the patch works for them, but cold turkey means they used their will power to not ever use tobacco again.....(good for them! :-)

Like suddenly and not gradually. For example, if someone wanted to quit smoking cold turkey, they would throw away all of their cigarettes instead of slowly stopping, like limiting it down from two packs a day to one. :)
