decisions in - life, future? are based on mistakes that has been done in the past? i'm kind of confuse, please explain more. thanks.

Decisions in a BUREAUCRACY are based on what has been done in the past.

A perfect manager performs by the book - process not product - ISO900x, Six Sigma. Official Truth is that once the perfect hole is dug it makes no diffence what is placed into it. Success is inevitable and inescapable under perfect management.

Detroit is massively managed. It clearly needs even more oversight.

You have it partly. But those decisions are also by what can be done in the future. We not only react but we also act. And that can be a spontaneous and unpredictable activity that shapes the future.
