You could be honest but that would hurt the parents feelings. so if you said "what a cute shirt or outfit or shoes" that the baby has on, this might distract them away from you not saying anything about the looks of the baby. It is worth a try. If the baby has a pretty color eyes, then say I love his/her eyes. Try to play it safe.

Reading your question made me remember back in 9th grade...

I lived this! The front office was looking over a co-workers new baby....not with animation. When I came out to see the little tyke I squealed, "Cathy, he looks just like Yoda!" The room gasped and a huge smile came on her face. She said, "Finally, an honest remark." Even the mom thought him less than pretty and she kept track of who was lying about it.

Nice eyes...Naw that is what they said to me and I think I did not turn out so bad....Im Cute....I think I am ...Look at the parents add .. that you make gestures of cute and tell them lies to make them feel good and Just back out of any crack comments....IT will pass over and as the child gets older it kind of works its way out Beauty is in the eye of the beholder

I was at another girl's baby shower when I saw this girl who isn't that attractive herself with her child. The baby can't help it but whoa....what a sad little sight. Lucky for me I didn't have to talk to the girl.
