A nuclear accident occured in Tokai, Japan 30 September, 1999, here is the article: http://www.irpa.net/irpa10/cdrom/01329.pdf
There have, however been several near misses. Here is an excerpt from a website: On June 27, at 1:37 pm PDT, a wildfire started at the Hanford Nuclear Reservation in Washington state, about 120 miles from Seattle. Thousands of people were evacuated, and fire scorched nearly half the 560-square-mile site, destroying 70 homes and buildings as it crept within two miles of some of the most lethal nuclear waste on Earth—waste dating back to the creation of the atomic bomb.
Los Alamos and Hanford have also had some near-misses. Nuclear energy is a good thing only if vigilance is at it's optimum peak.
There was an incident in Harrisburg Pennsylvania, otherwise known as Three Mile Island. Chernobyl , Tokai. These were all major hits in nuclear accidents.

The last one would have been the Chernobyl plant in Russia in 1986. If you mean CURRENT, what is not talked about is all the depleted uranium in the Middle East because of the nuclear warhead being used--it has contaminated everything and everyone and that was also what Gulf War Syndrome is. It's very dangerous because of the radioactivity levels. It is also sterilizing the soldiers and others there, and will cause DNA mutations.

Chernobyl is the only real disaster that I can think of in recent history, though there have been tons of nuclear accidents in lots of countries. The number of deaths in the accidents has been small, however, and the result of the leaked radiation won't be known for some time to come. Here's an example:

There's a timeline here:
