Men will self relieve if they havent done it in ages. So you will never be able to tell that way.
Why dont you ask him outright?
If not go for the sneaky method of looking through his phone and spying on him!!!

You can't tell by physical examination... unless you get his underwear tested for remnants of a partner. You know.. that's going a bit far isnt it? You can get yourself a Private Eye.. or call cheaters..... or just ask him. Anyway, if he hasn't had it for a long time... is your relationship that close that you should be concerned if he sleeps with someone else?

the only way you could probably tell is by the partner they are with. if someone for example was going after hillary or our beloved speaker, my guess is theyve been out of ciruclation and have been in fact desparate for a very long time.
