I guess in his mind, he cares for the mistress more than his wife. And while he is having sex with his wife he feels guilty about it.
Or he is just telling you a line to keep you around.

He is an *** he didn't get married to keep a mistress he got married to be true to his wife give him the boot if this referrs directly to you. Get better there will always be a better man around than that..

It sounds like he thinks that his mistress is more important, and that it's unfair that she has to share him with his wife.
(Even though cheating on his wife is wrong.)
