A leap of faith is a burst of inspiration or understanding. It's called a leap because logic and common practice prescribe a very different course but a novel idea or solution is pursued instead. Something often contradictory to common sense but it works. If it fails it's just called stupidity.
Blind faith is the unshakable belief in something/someone. It can be good or bad, though the term is often used to describe negitive aspects of faith. Jim Jone's followers for example exibited blind faith in Jim Jones and many willingly drank poison because he said it was the right thing to do. The most recent tax cuts to the rich by Bush is another example. Instead of understanding the principles that made similar tax cuts by JFK and Reagen so successful Bush executed a tax cut not realizing that he instead of creating jobs in America just created more jobs in India and China as the economic dynamics had changed. Bush exibited blind faith in that he followed a tried and true tested plank of the Republican party. Instead of analyzing he acted on tradition and faith in his parties reasoning. A reasoning that was quite accurate 20 years ago when it was adopted but that was no longer really accurate.
Faith is a generic term that includes many kinds of faith. You listed two, but there are others. Inspired faith for example. Somebody who sees a religious event/miracle will be inspired and base their faith on a mystical event. The same thing is shown when for example a kind person walks into an orphanage and does a nice thing for an orphan. By changing one child's world they gain faith that kindness will change everybodies world. Faith is simply a belief held that disregards logic and analysis. It often embodies tradition and mystical experiences. It can have logical backing or it can be completely against one's logic. The key is that it is generated from emotion and or experience rather than analysis. Most beliefs are generated first from analysis. Then experience and emotion enter in.

"Faith" is believing in something even though it cannot be proved.
"Leap of Faith" is like playing a hunch or doing something based on a "gut feeling", without knowing all the steps that led you to a conclusion. It may be right or wrong.
"Blind Faith" is believing something even when everyone tries to show you are are wrong. Sometimes they are shown to be wrong and sometimes you are.

blind faith is faith that starts out as blind and ends as it as well. a leap of faith is taking a look over the tall mountain, 100 stories high then covering your eyes and jumping off the cliff, FALLLING, FASTER AND FASTER.
