The prcedure is always free. You just print a picture of a certain size. But the equipment to create the mug must be purchased, whether you are printing directly onto ceramic, or inserting paper between plastic walls.

If you want to know how to make personalized photo mugs, I think that it takes some special equipment and software for your computer. I am sure you would get the instructions free with the equipment. I have seen it being done at fairs and flea markets and those doing it have a special set up for their computer with camera, and printing equipment which will transfer the photo image onto just about any surface. Mugs, t-shirts, etc.
The website source I found does give detailed instructions. It is from the Home and Garden TV website. Does tell you what equipment you need also.

Like the other answers have said there are various sites for creating the photo mugs. I just wanted to add that while I haven't ordered a mug, I just received a photo book that I designed on Shutterfly and the quality is excellent. I couldn't be more pleased. The mugs shown here look to be very high quality if you happen to be interested in this type.
