Lack of communication from top (executives) to bottom (worker bees). Too many chiefs and not enough Indians. Management out of touch with workers. Management needs to manage by walking around and stay in touch with the employee. Management needs to keep its ear to the ground to really know what is going on in the organization.

Upper management tends to treat employees as cogs in the wheel. If they break down for any reason, the employee is immediately replaced. There's a lack of understanding by upper management that people aren't machines and can not be treated as such.

Lack of organization - no one has a clue about what is going on.

Most people want to be the "chief", no one wants to be the "Indian."

People become apathetic, hostile and withdrawn.

I see a lot of "stuff" falling to the ground. Clean it up. All can now blame everyone else for the failure.

1. Lack of professional management. Management selection in my organization is nothing about skill. It's all about being friends with someone higher up.
