George Burns always looked like he was smoking a cigar, because he was. Pretty much until he couldn't puff anymore, he smoked a cigar. I saw in an interview once that he just enjoyed them. Back in his day, smoking wasn't considered the same way as it is now.

It was his shtick. Burns had great timing, and he would use the cigar for that, setting up the joke, taking a draw on the cigar, and delivering the punchline

It was real and he was really smoking it...it was his trademark.
He was an actor when it was popular to smoke and later it was more a part of his character than anything else.
