how do i apply for short-term disability while i'm on maternity leave? it's supposed to pay half, or in some cases 100% of your salary for 6 of the 12 weeks you take off from your company. do i ask my company to apply me for short-term disabilty or does the state do it?

I am doing the same thing, you have to go to Human Resources, or if your co. doesn't have HR go to ur sup and ask about short term disability for maternity leave. For example in ND short term disability offers up to 60% of your pay up to $1,500 per week or flat rate of $300/week for 6 weeks

Yes. The company you work for should have an Insurance/Disability department that handles this sort of thing, whether you're pregnant or not.

I was granted intermitten leave during the first trimester and part my second due to severe morning sickness, and I gathered the paperwork through my supervisor who in turn gathered it from our Disability department.

Do you qualify for FMLA (Family Medical Leave Act?)? I know it maynot pay you for your time off, but if you do qualify for this (usually if you've been with your company for a year or more) this may hinder your chances at getting short term disability through your employer. This may be something you might want to bring up with your supervisor or manager.

Well I don't know how they do it at the job or state that you are in but at mine you have to have had purchased short term disability with your insurance before you become pregnant. I couldn't get it while I was pregnant because they said it was a "preexisting condition". So I don't think you'll be able to get it. You need to check with your job though before you take my advice.

I think I applied throught my dr's office (its been almost 6 yrs, it gets foggy). Ask your HR rep at work, they will have more info. Or contact the state dept of disability/social security.

I was told to get it through my docs office.
