If so, is there any provision for extra innings besides the commissioner calling the game?

No official limit but they try to keep any one pitcher from going more than a couple of innings. This is so they can pitch most of the guys and not adversely affect any one team after the all-star break. No team wants there ace to pitch 6 innings in an exhibition game and miss a real start.

No - no rule about how many innings a pitcher can work on the All Star game.

There is not, but it has been years since a pitcher has gone three innings. Most guys will go one innings with the starter and maybe one other going two.
