I saw it on late night TV about 30 years ago and it was a bizarre horror film set in a mental institution ( I think) and the in the scene I can't forget, the inmates begin attacking either one another or the staff in a surreal, orgiastic riot of mutilation which involved a close-up of someone's eye being removed and placed in a martini glass. I stopped watching at that point and never learned the name of the movie. I don't want to watch it....I've just always been curious about the name. It was probably an older movie even when I saw it. Thanks for any ideas.

One Flew Over the Cuckoo's Nest?

I have never seen the movie though, so I don't know about the mutilation scene, that wasn't in the movie... but it's possible.
Was Jack Nicholson in the movie?

Upon arrival at a mental institution, a brash rebel rallies the patients together to take on the oppressive Nurse Ratched, a woman more a dictator than a nurse.

Steve's answer is the most likely one, given its 1973 release date and the gore involved---another candidate is the 1972 Amicus release, "Asylum", starring Richard Todd, Peter Cushing, et al--an anthology of four grim & grizzly tales, which to my recollection, had some pretty nasty stuff here and there...based on stories by Robert Bloch, with a Poe twist...sorry I can't be of more of an assist.

definitely sounds like House on Haunted Hill
I haven't seen the 1959 version, but the 1999 starts off with killings in a mental institution (in a flashback)

It sounds very similar to a 70's movie, "Don't look in the basement". A real sick and gross b movie.
