You have to have an undergraduate degree, and med school is one of the fields where your major matters. Many medical schools will be reluctant to admit you if you have no experience with anatomy, physiology, etc. They want to make sure you can handle the subject matter (i.e. that you're not squeamish and will actually succeed).

Yes, you are required to have an undergraduate degree to apply for medical school. Your friend will need to get an undergraduate degree in some sort of physical science like biology or chemistry, and then take the required MCAT (Medical College Admissions Test).

You can't apply to Harvard Med School without a degree.

Your friend should be aware that about 15,000 well-qualified students apply to Harvard Med every year and about 200 are accepted, so he is setting himself quite a challenge.

On the other hand, 200 kids DO get accepted, so there is always hope.
