Okay when someone draws a picture and says in the description
Pen, ink and white acrylic on paper used. Like in this picture.

Are they literally using a pen for the drawing? If so what one, and how are they not messing up while drawing in pen and how are they shading so well?

The pen is probably an old-fashioned-style dip pen used with India Ink, or a fountain pen. It could also be a pigmented fine-point marker such as a Copic Multiliner or Pigma Micron.

The shading can be done by: using a very fine pen tip & drawing faint lines to simulate shadow areas; adding water to the ink & using a paintbrush to do the gray areas such as the grassy spots in the forground; by using a watery mixture of white acrylic to bring back the highlights.

It's possible that the artist used pen over top of a pencil drawing, then erased the pencil after, but many artists can draw like this without a preliminary pencil drawing.

Yes they use an actual pen
and you just have to practice practice.
