the phone number I got has a 4 digit extension number. Do I replace the last 4 digits of the regular number with the 4 digits of the extension number?

JUST DIAL THE PHONE # AND WAIT UNTIL IT PICKS UP THEN ENTER THE 4 DIGIT EXTENSION NUMBER

Dial the number. When someone picks up, tell them to connect you to the extension number.
