If a woman has a child in care and is pregnant a case conference should be held to discuss the future of the unborn child and an unborn child can be placed on the child protection register

In most states as a nurse midwife you are a mandated reporter. Which means quite simply, that you are required to report any suspected child abuse to the dept. of children and family services.

I don't know what the laws for midwives might say, but the logical course of action seems to be contacting the local/state government's child welfare division about your suspicions.

Report both verbally and written to your superior. If this has no effect you can phone the NSPCC and report to them. Social Services can also be contacted.
