Have to agree with mimik on this - sounds like a water quality problem. These are huge fish and need an equally huge tank to dilute their wastes.

Loss of appetite, lethargy are both symptoms of ammonia and/or nitrite poisoning, cloudy eyes (may be bacterial rather than fungal) and fin rot are related to poor water quality as well. As he's growing he's producing more wastes, and if you've been treating the tank with any antibiotics, these kill the beneficial bacteria that convert the ammonia and nitrite (both toxic to fish) to less harmful nitrate.

About the best you can do for him is to increase the size and frequency of water changes and get the largest possible tank you can to keep him.

Test for ammonia and nitrite. Both should read zero. What size tank do you have? An 18" Pacu should be kept in no less than a 75-100 gallon tank. A 225 would be even better although it will eventually out grow that too.
