usuall they just ask the first day of your last period. It's an estimate unless YOU know when you concieved. If you are looking for what your due date will be, they usually calculate it from the middle day of your last period and then once they do an ultrasound and take measurements your due date can change.

You might already know if you have the date your last period began. That's what the doctor uses to figure your due date. He takes the date of your period and figures you ovulated/conceived around Day 14 or 15 which is based on the average cycle.

So if you wanted to figure you when you conceived, add 14 days to the day your last period started. That's a rough estimate, because every woman's cycle is not a perfect 28 days, but it will give you a good idea.

If you know the first day of your last period, then you will be able to find out beforehand.

Go to Google.com and search due date calculators. Most of them will tell you your due date, date of conception, and how many weeks you are.
