In short - yes. Unless your own car insurance will cover you if your driving another vehicle. Generally when we rent a car, we are either driving a vehicle different from our own, or are in a different area than we would normally be in, or both. Both increase the risk of an accident. Better take out a little insurance, than to find yourself greatly out of pocket, should the worst happen. And it doesn't even have to be your fault. My husband's rental car was broken into in a beach side car park, and the window and door were damaged. Without the insurance, that would have been the end of the holiday for him.
