Even if you don't have a Medical Doctor you can still retrieve copies of your records. What ever hospital you were at last should have a copy of your medical records which includes all immunization records, testing and general physician case notes from appointments. You may want to call and ask for there Medical Records Department. For legal reasons they may want you to come in and sign a medical records release authorization form or they may be able to mail it (depends on the hospital) there may be a per copy/page fee, but nothing that will break the bank. If you must sign a release form you may have to wait a few days before they release the paperwork to you.

You send a request letter, or call your previous doctors or medical facilities and ask to have a copy of your records. You may have to pay a small fee though.

That does not work all the time if you move from one town to another and forget to take your med records with you your former doctor or hospital will destroy your med records after a period of time.

in the united states you can get your records from your old doctor its called the freedom of information act. Canada im not sure but call your local government agency. ask them how you can get them sorry i couldnt be of more help

ask the hospital you were born at, they should have the health records for you
