One thing that helped me is to have a timing mechanism that kept me lose at a plate. If you're physically lose and not tense, it's easier to stay mentally focused. I slowly swing my bat in front of me like a golf club (sort of like Jim Thome) then right before the pitcher winds up the bat goes up to my shoulder. Find something that makes you comfortable.

Try to find the timing for the pitch while in the on deck circle. Take a pitch, stride as if swinging to tune timing and develop vision for the pitchers release point. On the next strike, make solid contact.

I tend to think of a great line (one of many) from the best baseball film ever produced, Bull Durham.
Nuke is nervous because his father is at the game, filming him. Crash comes out to ask what the problem is? Nuke mentions his father, pleading Crash not to look.
Crash turns, waves to Dad. Nuke gets even more flustered.
Crash: "So it's your old man, so what? He's as full of $#!+ as anyone else."
Remember that. Everyone is full of it. The pitcher. The coaches. The umpires. YOU. Everyone. Then find your pitch.

take a deep breath, stare hard at the pitcher and ball, and run this message through your head "I'm Gonna Hit The Ball", oh, and if you are afraid of getting hit, I've been hit by 6 pitches, never got hurt.

Focus on JUST
-SEEING the ball......
HITTING the ball.......
ITs too late to worry about the specifics when already at the plate.
Chew a piece of gum... IT relieves stress. That's why they give you a piece before the SAT
