basically i need the code for a form that does the following;
1) contains the following user imputted fields (name, Address,receive newsletter tick box,e-mail address AND An atttatch file (3gp,avi,mpg)
2) it hen needs to e-mail me the information and teh file to contact [at] omnic.co.uk
i know you need to use sendmail but im confused! help!

www.jotform.com gives you an online form editor: you develop the form, using his editor, adding and/or subtracting form fields, to build a form (even a library of forms, if you wish) for your website. Then you either link to that form or copy the generated html to your website and style it as appropriate for the look and feel of your website. The resulting form sends output to the jotform server which then forwards the form output to an email address of your choice. It's all free. Generous, que no?

You need to get someone to develop a PHP page for you.
The client or whoever is filling out the form can fill it out online and then that page with all the copy is directed your email box. Very easy limited cost about 250.00 dollars.

It works great and creates the form in a variety of languages.
Only problem I have ever had is one of my hosting accounts does not support the DLL required for it to work.
