I would do it like how rappers who are in movies have their real names and in the middle in quotes is their rapper alias.
for example:
First Name "Nickname" Last Name
anyway Your name is written on a golden gate in heaven, or so I'm told.

I wouldn't have to sign my name in the book of life. My name would already be in there, thats how I would get accepted into the pearly gates.
