How do you move an elderly relative with dementia, severe vision issues and depression from one assisted living facility under VA insurance to another state's (Florida) assisted living facility? What do I need to know? Please, help me. I am a caregiver and my extended family is at war over this. what documents/exams/laws do I need to know?

Yes, she is a Veteran. I meant VA for Veteran not Virginia. I am awaiting to hear back from a VA social worker before I can even begin this process or whatever it is going to be.
All of her doctors there told me the only way to get in touch with them is this one PAGER number. I leave my number and have no idea when they will call back or who will be calling back (i.e. the psychiatrist, the social worker, the eye doctor, the pharmacist..
Any thoughts on family split on decision but some members have more monetary power than others? Attempting to move from Mass. VA to Florida VA as far as doctors and health insurance.
I cannot thank those of you with answers enough in advance.

My thoughts are with you and your family. I cared for my mother the last 5 years of her life, her mind was about the level of a two year old.

Is the relative a vet? and moving to a VA facility, or is that an initial for Virginia? If the person is a vet, contact a counselor or social worker at the Veteran's Administration hospital in your area.

It's a shame the person has to suffer with depression with all the other problems. Hopefully medication of some sort can help with that. Maybe you could make sure the relative is visited by facility volunteers?

At this point, it would be more appropriate to place him in a nursing home rather than assisted living because of the dementia and blindness, as he will require more care than assisted living can provide.
There are nursing homes that have contracts with the VA (as you know by now, I'm sure) as well as nursing homes on many VA hospital campuses.
I would call the nursing homes in your area and talk with admissions counselors to see if any have a VA contract. If they do, that will narrow down the number of facilities you will need to inspect, and make your job easier.

Does anyone have health care power of attorney over the relative? If so they are the only one that can move them. If not then who signed them into the facilty? Does the realtive have any decision making ability left? If you are the POA of the resonsbible party then you will need to look at other facilities decide on one you like and ask them to look at your loved one for placement and if they are appropriate for the facility. YOu will also need to inquire about the VA payment acceptance.
Usually there are doctor forms that have to be completed prior to admit. You need to check with the current facility somehave notice policies.
You can also look into hiring a geriatric consultant who can look into the facilities
You may need to file for guardianship to have your realive ajudicated incompentant this will allow you to make the decisions

Your best bet is to contact your local Alzheimer's Society (most handle all forms of dementia as well) and speak with Client Services. They were very helpful in giving our family advice about my grandma.

I can't help you with the documentation, but before you move her, ask her physician for some tranquilizing medication to help her cope with the move. Dealing with dementia and depression can be very difficult.
