It may differ from state to state but this much I know in Washington state.
You either have to be over 18 or you have to have consent from both your parents, whether they are married or not.

For information in your state, I suggest calling the capital city hall.

The specific answer to your question depends on which state you live in. Check your State Statute on name changes. In Nevada, where I live, you have to file a petition in the family court, and then publish a notice in the newspaper for a few weeks. You can't be a convicted felon, and you can't be doing it to avoid creditors, or for any other fradulent reason.

Okay, two answers here.

One, you can use any name you want, as long as you aren't using it to defraud anyone.

Two, you can legally change your name at the County Clerk's office (Town Clerk in two states) by filling out a Name Change Certificate, and paying them to Notarize and Witness it, and paying the appropriate recording fee.

You can (it's easier if you do it when you get married, but you can randomly do it too)

You have to go down to the city's courthouse (where they do all the office work and papers)

Usually around 50 bucks, but it can get more expensive.

Then after it's changed there you take those papers and go to the social security office that's nearest to you and change it there (you'll get a new card etc.)

And you also have to change it everywhere else, like your local DMV and your city building (Like where you get your water and have your sewage and trash collection stuff go on)

Your workplace has to change stuff for you too, you have to redo all your tax W2's and other papers.

Your bank too..

It's kind of a hassle, but if you really want to change it go for it!

You'll need to fill out several forms, especially one to the Social Security Administration

Depends on how you go about it, you can have a service do everything for you for a fee/charge. OR you can do everything yourself, and pay the fees as you go along.

Yes, as long as your over 18 years of age and not a fugitive from justice or on parole.

In Florida it is done at the clerk of the court inside the county courthouse.

It does not take much paperwork...

and I don't know how much it costs...

You can legally do it, it costs like $500 or more
talk to department of motor vehicles they can point you int he right direction as to your next step.
